What is ‘Foundations’?
The Foundations program helps young people transition successfully from school to adulthood and develop skills that will help them achieve their career, volunteer and community participation goals.
What services are available through Foundations?
Foundations supports families in navigating developmental services, holds public information sessions at least 2 times per year, AND connects people to resources for accessing funding, community programs, respite, and other government and community services.
Life After School Planning
Foundations is a resource for individualized transitional planning and supports young adults in exploring personal interests, identifying goals, developing skills, overcoming barriers and building a meaningful future.
Who can apply to participate in Foundations?
The Foundations program is for young adults with a developmental disability who are in school OR who have left school and need support in creating a plan for the future.
How can I access Foundations?
You can apply to receive ministry funded support through contacting Developmental Services Ontario (DSO) by phone at 1.855.277.2121 or by email at email@example.com. After reviewing your application, DSO will make a referral to Community Living Campbellford/ Brighton, identifying your interest in Foundations. To learn about the details of the application process you can also visit the DSO website at www.dsontario.ca.
What are the possibilities after secondary school?
Foundations empowers young adults to define their independence and develop skills such as resume writing, computer and interview skills, literacy, money management, travel training, community safety and more.
Foundations can help locate career options and provide support in mentoring and on- the-job training. Potential jobs may include: courier services, maintenance work, retail sales, laundry and dry cleaning services, food services, packaging and assembly and administrative work.
Foundations offers guidance in exploring volunteer opportunities such as visiting people in hospitals, delivering Meals on Wheels and assisting at day care centres.
Foundations helps young adults develop the skills and confidence to become involved in community activities and use public services such as educational and recreation facilities.
