Foundations is a Four county partnership designed to support people aged 21 – 28 with an intellectual disability make a successful transition from school to their community.
Foundations supports include developing community connections, assistance with employment goal’s teaching daily living skills or pursuing post secondary education. All supports are directed by the person using the Personal Outcome measures as a guiding tool.
Foundations offers transitional planning to students with an intellectual disability while attending high school and immediately after.
What is involved in the planning?
- Co-ordination with home, school and other resources
- Recognizing what the student wants in his/her future
- Connecting with activities and experiences outside of school
- Recognizing and developing personal strengths and interests
