Community homes provide community living options for people with intellectual and physical disabilities. The focus of this support is to provide people with the opportunity to enhance their skills and to be active and included within their community.
Outcomes Support Facilitators promote and encourage healthy and positive lifestyles and assist people to pursue their own interests and dreams. Individual choices, family involvement and Personal Outcome Measures determine what supports will be provided. Community Living Campbellford Brighton currently supports four homes, three in Campbellford and one in Warkworth.
